Important:

  • The purchase order is mandatory. If you request services, you are obligated to provide a purchase order.
  • Our agency cannot be paid for services rendered without this documentation, per DHR regulations.

Service Request Policy

To provide services, the following requirements must be met:

  • A completed Intake Form must be submitted.
  • A valid Purchase Order (Form 1878) must follow every service request.

Important:

  • The purchase order is mandatory. If you request services, you are obligated to provide a purchase order.
  • Our agency cannot be paid for services rendered without this documentation, per DHR regulations.

After-hours requests:

  • If a purchase order cannot immediately be submitted, the intake form will be accepted.
  • The purchase order must be submitted the following business day without exception.

By submitting a request, you acknowledge and agree to these requirements.
